1. Open the "Report a Malfunction/Near Miss" module.
2. Click on the "New Report" button.
3. A form will appear where you can select the type of your breakdown/near miss, set the priority, and provide a detailed description of your issue. You can also attach up to two photos. After completing the form, click the "Submit" button at the bottom of the screen.

ADMINISTRATION IN BACKOFFICE
1. Open the "Report a malfunction/Near-Miss" module.
2. Select the "New Report" form.
3. Here, you can view all the completed forms. To edit individual reports, click on the blue "Show" text.
4. Then click on the "Edit" button.
5. You will be directed to a form where you must specify the status of the report (so the employee receives a notification that a response has been sent) and write a response to the employee or a note for the administrators. You can freely edit the breakdown/incident details, change the priority of the issue, and also add, change, or remove a photo (with a maximum size of 10 MB). After making edits, click the "Update" button.
6. You can also export all reports into an Excel spreadsheet or a PDF file.
7. If you plan to edit your table and then upload the changes back, please first save the Excel spreadsheet using the "Export CSV" button. You can make any changes in the Excel spreadsheet, but it's essential that the ID and Created columns are filled out, as they ensure your changes are saved correctly.
Once you've made all necessary changes, please save the file and then import it back using the "Import" button. A window will appear informing you that only the columns containing ID and Created information will be saved. Please verify that all data is correctly entered, then press OK.
